Trident Basketball Registration

Please download and PRINT the registration form. Fill it out and turn in WITH a copy of your athlete's birth certificate as well as a picture stapled to their registration form. If the picture does not fit then please try a different picture. We cannot provide you with a copy of your athlete's birth certificate.

If all three requirements are not met then your athlete's registration will be invalid. 

This is for boys grades 5-8 and girls grades 5-7.
